I don't know if this is a widespread issue within the community, however I have an issue where I literally forget who's who in my friends list.

I can meet you one day, have a great conversation with you, add you and then totally forget about you the next day. This happens to me frequently (almost on a daily basis) and used to happen to me on POTCO as well especially when I stopped playing for a while. My friends list becomes a list of random names.

As most pirate names and looks are quite generic, it's pretty hard to remember who's who. Hence why it would be interesting to see a "notes" tab embedded in the friends list where pirates could actually take note of who said person is and why they added them. It could be a *memento-like situation.

Alternatively, a friend ranking system could also do it.

TL;DR:
Having a method of being able to remember who my pirate friends are would allow for a more long-term social experience within TLOPO. 1st world problems, I know :'(

This happens to me on other games (I don't have many friends on TLOPO).

I have the same problem, given it was someone I only casually spoke to and they aren't an *actual* friend. As the list grows longer, I begin to forget how I met the person and what I did with them. League of Legends has the system you are talking about -- you can add a sort of "sticky note" to each person. I employ this method to remember who my friends are on that game.
 
There is a lot going on in the world and peoples lives. I think it is not bad if we forget interacate details about the characteristics or even names of those we have met and friended in a mmo game.
I have few friends but i choose them carefully. I like to talk and get to know them first (no personal info just character)
If we have fun I most likely will be friends and hope to catch them again online. If they are not very active and I meet more people I regrettably forget about them.

It's just how things go :)

Another interesting idea would be to have the game note down the chat between you two and where you both were around the time that you became friends. This would be a helpful extra step to automatically take note of who people are.
 
This happens to me quite a bit. But typically I use my friends list to ask for help when I need it and just send random messages to everyone online until I can find help. Of course the guild is usually more helpful for that anyway. On this game I have been able to notice patterns in what certain people like to do and am able to remember each person based on what I wind up doing with them most.

And it can be quite fun to get the random whisper from a friend that seems to remember you and try to figure out who they are without letting them know that you are completely clueless.
 
It be cool if we also have a way to mark those "friend" that's actually people we want to hang out next time and not a casual acquaintance.

This sounds great, until you're captaining a ship and you befriend people who crashed so they are able to get back on (easily).

Then you think to yourself "I'll just unfriend them after they get back" but you get distracted by that broadside from Battle Royale that almost hit you because you were distracted. Next thing you know you're ramming your way across the ocean with nothing on your mind but keeping your ship and crew afloat.

Two days later you're now me and you've always got two pages of people online but you only know who three of them are.
 
This sounds great, until you're captaining a ship and you befriend people who crashed so they are able to get back on (easily).

Then you think to yourself "I'll just unfriend them after they get back" but you get distracted by that broadside from Battle Royale that almost hit you because you were distracted. Next thing you know you're ramming your way across the ocean with nothing on your mind but keeping your ship and crew afloat.

Two days later you're now me and you've always got two pages of people online but you only know who three of them are.
Or you walk by someone, they say hi, and you say hi back, and then they friend request you. You feel bad for the poor, low-leveled soul, and you just can't say no. Fast forward two days and you have no idea who the person even is that's clogging up your friends list.

Yeah why do I feel so bad about declining friend invitations in POTCO / TLOPO when in other games I'll decline basically all of them?

For that matter, how did I go a couple weeks declining every guild request immediately but unable to decline random friend requests without staring at it for three minutes first?

I never accept those. If someone asks to be my friend for no reason and I haven't said anything to them, I'm declining. I've done that at least a dozen times and never had any confrontation about it. But, if I board someone's ship or they board mine and we have a little conversation and they try to add me, then I probably will accept. If someone gets a cluttered friend list because they're adding people they don't know left and right, that's their own doing.

That being said, I also have an issue with adding people and not remembering when or why I did so. But OP's suggested fix isn't really the kind of thing I could see being put in. If you need notes about the people you add so badly, just use another application on your computer or something.

The same thing happened to me, I added some random low-level pirate to my crew and they immediately accepted and friend requested me. I couldn't decline the poor kid's friend request.

I keep on incessantly doing that, too. There should be a "recently met" or "acquaintance (but not friend)" list.
